We would like you to fill these forms out and bring them with you. Or, if you prefer, the link below will take you to these forms, which you can print from your computer, fill out and bring with you to your first appointment. We ask all of this information to help us be as complete as possible in knowing your individual situation, so we appreciate the help you give us!

Dental Insurance

Insurance plans are designed to provide an average reimbursement amount for average quality care in patients whose mouths are in average health condition. If a patient has a very healthy mouth, they will not use up the benefits their employer pays for. If a patient has an unhealthy mouth, the dental work needed to restore them to health will exceed the benefits the employer has paid for.

We make the recommendations for treatment needed by our patients based solely on their oral conditions, and never on whether or not they have dental insurance coverage. We make every effort to educate our patients as to the conditions that exist, the treatment that is recommended and alternatives to that care. We want our patients to make the most informed choices possible, and provide them with the best possible care.
